- Installed for a driver or passenger with a permanent disability
- Purchased from and installed by a licensed equipment installer

The purchaser and a General Motors dealer representative will need to complete and sign the reimbursement application. Submit the application with a paid receipt for the eligible accessibility equipment and installation.

MORE TO CONSIDER
Here’s how to get started when considering adaptive equipment
CONTACT YOUR STATE DRIVER’S LICENSING AGENCY
Check with your state driver’s licensing agency for current driving regulations.
CONTACT A DRIVER EVALUATOR FOR AN EVALUATION
Before selecting a vehicle, consult with a driver evaluator (Certified Driver Rehabilitation Specialist or CDRS). Your CDRS will evaluate you and assess your driving abilities, suggest driving aids and determine if you need to have your driver’s license modified.
Note: You will need to leave GMFleet.com. General Motors is not responsible for the content provided by third-party sites.
CONTACT YOUR LOCAL LICENSED ACCESSIBILITY EQUIPMENT INSTALLER
When having any aftermarket device installed in your vehicle, you’re responsible for contacting an independent licensed accessibility equipment upfitter† to have it properly installed. A good place to start is the National Mobility Equipment Dealers Association (NMEDA). They are certified through the Quality Assurance Program, a nationally recognized accreditation program for accessibility equipment installers.
